I guess the harm could be in terms of revealing personal information. Also if its a clinical study a person's confidential medical information can be revealed, in case
proper measures for protection of the human participants are not taken.

Yes, harm can happen to a participant in two ways - direct harm from participating in the survey and harm from confidentiality breaches.
